Thousands of pages. The clinical detail that matters. Found in minutes.

LexGuard is an AI-powered medical record review platform built for physician expert witnesses and litigation teams on both sides of the aisle. Upload the case file. Get a cited, searchable intelligence layer over the entire record. Switch between analytical perspectives with a single click.

The Problem

Record review is the bottleneck on every side of every case.

Plaintiff counsel needs to find deviations from standard of care buried across years of clinical documentation. Defense counsel gets the records last, reviews them under tighter deadlines, and must find what the plaintiff's team missed or chose not to highlight. IME physicians face stacks of records from a dozen providers and must synthesize them into an objective, evidence-grounded determination within days.

Every one of these workflows begins with the same bottleneck — reading, organizing, and cross-referencing thousands of pages before the clinical thinking that requires a physician can begin. LexGuard automates that foundation so the expert can start where their training matters.

The Manual Review Reality

25–30 hrs

Typical physician expert time spent on manual record review per case

8,000+ pages

Average defense file size spanning multiple providers and years of care

$0 opinions

Value of clinical judgment formed during the indexing phase — before analysis begins

The Platform

Record review with clinical authority.

LexGuard reads the entire case record and builds a searchable, citation-backed intelligence layer over it. Every finding links to its source page. Every answer is grounded in the documentation.

Ingest and Organize

Upload thousands of pages as PDF or pull records through FHIR. The platform classifies every document by type, extracts text with OCR, processes clinical images through vision AI, deduplicates overlapping records, and generates a master chronology with significant events flagged — all within minutes.

Analyze and Query

Ask clinical questions in natural language and receive answers grounded in the case record with page-level citations. Gap analysis, standard-of-care evaluation, causation reasoning, and provider comparison. Switch between Plaintiff, Defense, and Objective modes at any time.

Report and Prepare

Expert opinion templates auto-populate from chronology data, bookmarked exchanges, and annotations. Export as Word or PDF with embedded citations. The testimony module generates anticipated cross-examination questions and adversarial rehearsal for deposition practice.

How It Works

Four steps from upload to opinion.

01

Upload Records

Batch PDF upload or direct EHR pull via FHIR. Single files or ZIP archives from any source in any scan quality.

02

AI Processes

Classifies documents, extracts clinical content, deduplicates records, and builds a master chronology with significant events flagged. Minutes, not days.

03

Query the Record

Ask clinical questions in plain language. Every response cites specific documents and page numbers. Switch between analysis modes freely.

04

Generate Reports

Opinion templates populate from chronology, bookmarked exchanges, and annotations. Export with citations. Build cross-exam outlines for deposition.

Most cases are fully processed and ready for expert query within 30 minutes of upload — regardless of file size, scan quality, or number of contributing providers.

One Platform, Three Analytical Lenses

The same record. The right perspective.

LexGuard uses the same underlying data and retrieval engine regardless of which side you represent. The analysis mode adjusts the analytical framework — the clinical intelligence adapts to the questions each role needs answered.

Plaintiff Counsel

Find what was missed. Prove what it cost.

Plaintiff experts must identify deviations from accepted standard of care, establish causal linkage between those deviations and patient outcomes, and quantify injury severity from documented findings. LexGuard detects gaps in follow-up, missed diagnostic opportunities, and delayed referrals across the full record set.

Defense Counsel

Find what they missed. Build what protects you.

Defense counsel gets the records last and bears the burden of finding what the plaintiff's team overlooked or chose not to highlight. LexGuard identifies pre-existing conditions, alternative etiologies, and documented inconsistencies between claimed injuries and contemporaneous clinical findings.

IME / Peer Review

Objective synthesis. Defensible determinations.

Independent medical examiners and peer review physicians must synthesize clinical history across all providers into a unified, objective timeline. LexGuard applies the Objective analysis mode to present findings without advocacy framing and generate structured reports conforming to payer or jurisdiction-specific templates.

The Economics

The math is immediate — on both sides.

Whether you are plaintiff counsel managing case valuation, defense counsel navigating billing caps, or an IME practice scaling throughput, the time savings translate directly to recovered capacity and reduced cost per case.

Frequently Asked Questions

What litigators on both sides want to know.

Litigation teams evaluating AI-assisted review tools have specific questions about privilege protection, disclosure obligations, and data security. These answers reflect how LexGuard is designed to meet those concerns directly.

Is the AI's analysis discoverable?

LexGuard is a record review tool used at counsel's direction. The platform's outputs are generated as part of the expert's work product. Discoverability depends on your jurisdiction's work product rules, but the platform supports privilege protection through matter-level isolation, audit trails, and certified deletion.

Do I need to disclose AI use to the court?

Disclosure requirements vary by jurisdiction and are evolving. LexGuard provides a documented verification trail demonstrating that the expert independently reviewed and validated all AI-assisted findings. The platform supports compliance with emerging disclosure obligations.

Can opposing counsel subpoena your servers?

The platform uses zero-retention AI processing. Case content is never stored by the AI provider after the query completes. All case data resides within your firm's isolated partition on HIPAA-compliant Azure infrastructure. There is no third-party repository.

What happens to case data after engagement ends?

Retention policies are configurable per matter. Counsel can request certified deletion with confirmation reporting at any time. The platform documents deletion in the audit trail. Default retention is seven years, adjustable to match firm or carrier requirements.

Can plaintiff and defense use the same platform?

Absolutely. LexGuard supports all three analytical perspectives through mode switching. The underlying data and retrieval engine are identical. The analysis mode adjusts the prompt framework and analytical lens. Matter-level data isolation ensures complete separation between cases.

Does LexGuard integrate with case management systems?

LexGuard offers API access for integration with Relativity, NetDocuments, iManage, and other platforms. FHIR-based connectors support direct record ingestion from EHR systems. Enterprise integration support is available for firms with custom workflows.

The Team

Built where medicine and litigation meet.

LexGuard was founded by a team of attorneys, a physician, and a technology entrepreneur with direct experience in plaintiff litigation, defense practice, medical-legal review, and healthcare AI. The platform reflects the workflow of physicians and litigators who lived inside this work before building the technology to improve it.

Every feature in LexGuard was designed by people who understand what it means to prepare an expert for cross-examination, to find the single contradicting note buried on page 4,200 of a scanned hospital record, and to deliver a defensible opinion on a tight deadline. The platform is not a generic document tool adapted for legal use — it is purpose-built for medical-legal review from the first line of code.
